Washington state disciplines health care providers | Seattle ...

Periodically Washington State Department of Health issues an update on disciplinary actions taken against health care providers, including suspensions and revocations of licenses, certifications, or registrations of providers in the state.

The department also suspends the credentials of people who have been prohibited from practicing in other states.

Information about health care providers is also on the agency?s website.

To find this information click on ?Provider Credential Search? on the left hand side of the Department of Health home page (www.doh.wa.gov).

The site includes information about a health care provider?s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998.

This information is also available by calling 360-236-4700.

Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Note: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.

State disciplines healthcare providers

OLYMPIA - The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our?state.

The department?s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency website. Click on ?Look up a healthcare provider license? in the ?How Do I?? section of theDepartment of Health website?(doh.wa.gov).

Benton County

In January 2013 the Unlicensed Practice Program notified?Colleen Patridge-Staudingerof its intent to issue a cease-and-desist order. On about four occasions Patridge-Staudinger injected people with botulinum toxin type A for cosmetic purposes. She doesn?t have a license to practice as a physician in Washington.

Clark County

In December 2012 the Secretary of Health withdrew the October 2012 statement of charges against registered nursing assistant?Amy Raelynne Comella,alsoknown as Amy Raelynne Ross (NA60277569).

In December 2012 the Physical Therapy Board released physical therapist?Gary J. Maguire?(PT00005702) from terms and conditions set against his license.

Douglas County

In January 2013 the Health Care Assistant Program modified an agreed order with?Jennifer Dawn Paiva?(HC00142564) and placed her credential on probation until she meets all conditions in the order. Paiva must provide a copy of the agreed order to any future employers and the employer must verify this to the program. She must also pay a fine.

Franklin County

In January 2013 the Nursing Assistant Program charged registered nursing assistant?Amanda L. Perez-Leon?(NA00106161) with unprofessional conduct. Perez-Leon was convicted of second-degree possession of stolen property in 2002, unlawful possession of a controlled substance (methamphetamine) in 2002, third-degree theft in 2008, and shoplifting in 2011. She allegedly also didn?t report the convictions as required, and stole $600 from a patient?s daughter.

Grant County

In January 2013 the Nursing Assistant Program charged certified nursing assistant?Ashly Virginia Eli?(NC60245997) with unprofessional conduct. Eli was convicted of first and second-degree criminal mistreatment in 2012 for severely abusing her two young children and not seeking medical help for them for about two weeks after the abuse took place. Eli was sentenced to a 10-year prison term.

In January 2013 the Nursing Assistant Program charged certified nursing assistant?Denis D. Mulamba(NC10089932) with unprofessional conduct. Mulamba was convicted of first-degree assault of a child, second-degree assault of a child, first-degree criminal mistreatment, and third-degree criminal mistreatment in 2012 for severely abusing his girlfriend?s two young children. Mulamba was sentenced to a 40-year prison term.

King County

In January 2013 the Massage Program ordered?Shannon Jeanne Beatty?(MA60274064) to re-enter a substance abuse monitoring program. Beatty must comply with all aspects of the program.

In January 2013 the Chiropractic Commission modified an agreed order against chiropractor?Mark T. Dang?(CH00033803) and placed his license on probation until he has completely paid his $10,000 fine.

In January 2013 the Dental Commission charged dentist?Edward S. Dolan?(DE00008771) with unprofessional conduct. Dolan allegedly didn?t provide treatment records to a patient upon request.

In January 2013 the Unlicensed Practice Program notified?Dana A. Klueverof its intent to issue a cease-and-desist order. On about 13 occasions, Kluever treated a patient with a laser and intense pulsed light. He doesn?t have a license to practice as a physician in Washington.

In January 2013 the Dental Commission charged dentist?Joan M. Randish?(DE00005671) with unprofessional conduct. Charges say that on more than one occasion, Randish provided dental services to a patient that didn?t meet the standard of care for a dentist in Washington. Randish also didn?t keep proper treatment notes, and didn?t update the patient?s treatment plan.

In January 2013 the Nursing Assistant Program amended the statement of charges against registered nursing assistant?Nita B. Simonelli?(NA60213648). Simonelli was fired from her workplace for stealing from a resident. The Department of Social and Health Services issued a notice of final finding based on the financial exploitation of a vulnerable adult.

In January 2013 the Nursing Commission charged registered nurse?Russell Stevens?(RN00133989) with unprofessional conduct. Stevens allegedly didn?t comply with a previous stipulation.

In December 2012 the Secretary of Health withdrew the August 2012 statement of charges against X-ray technician?Richard W. Young?(XT60000893).

Lewis County

In January 2013 the Nursing Assistant Program charged registered and certified nursing assistant?Miranda Mae Klemola?(NA60187635, NC60195622) with unprofessional conduct. Klemola allegedly yelled at a patient, used profanity, and pushed the patient down.

Pierce County

In January 2013 the Nursing Assistant Program entered an agreed order with certified nursing assistant?Alena D. Alves?(NC10045511) to reinstate her credential.

In January 2013 the Nursing Assistant Program charged certified nursing assistant?Cody L. Burnett?(NC10054798) with unprofessional conduct. The Department of Social and Health Services issued a final finding that Burnett abused several vulnerable adults. He can no longer work in a position that involves unsupervised care or access to vulnerable adults.

In December 2012 the Nursing Assistant Program denied the certified nursing assistant credential application of?Katherine Michele Dobbs?(NC60299886) because she didn?t respond to the notice of decision. In 2012 her registered nursing assistant credential was granted subject to probation based on a 2008 conviction for burglary.

In January 2013 the Counselor Program released mental health counselor?Shonna Lee Porter?(MC60298915) from terms and conditions set against her credential.

In January 2013 the Nursing Commission charged registered nurse?Micah W. Kaluzny?(RN00174016) with unprofessional conduct. Kaluzny was convicted of driving under the influence in 2012.

In January 2013 the Nursing Commission charged registered nurse?Christa Wandolowski?(RN00144181) with unprofessional conduct. Wandolowski allegedly diverted controlled substances from her workplace and didn?t comply with her substance abuse monitoring contract.

Thurston County

In January 2013 the Unlicensed Practice Program notified?Xia Liof its intent to issue a cease-and-desist order. Xia provided a full-body massage to a client and she doesn?t have a credential to practice as a massage therapist in Washington.

In December 2012 the Dental Commission withdrew the June 2012 statement of charges against dentist?Jin S. Miao?(DE00007830).

Walla Walla County

In January 2013 the Chemical Dependency Professional and Social Worker Programs placed the credentials of chemical dependency professional and independent social worker?Kim E. Funderburk(CP00005068, LW60020675) on probation for at least two years.

Whatcom County

In January 2013 the Nursing Assistant Program charged registered nursing assistant?Linda J. Smith?(NA00117236) with unprofessional conduct. The Department of Social and Health Services issued a final finding that Smith financially exploited a vulnerable adult. Smith can no longer work in a position that involves unsupervised care or access to vulnerable adults.

Yakima County

In January 2013 the Nursing Assistant Program charged registered nursing assistant?Erika G. Amaro?(NA60198158) with unprofessional conduct. Amaro allegedly didn?t comply with a previous stipulation
